1. Log in to your Zane Benefits account
2. Click on Submit Claim
3. Select the type of expense you're submitting
4. Enter the name of the provider, date of service, recipient of service, and amount billed. Then, add the Explanation of Benefits (EOB) to the request.
5. Check the box next to "I agree to the Disclaimer and Electronic Signature.
6. Click on "Submit for Review."
