Your ZaneHealth benefit reimburses you for your eligible health insurance premiums. You can also request reimbursement for out-of-pocket expenses for basic preventive care.

In order to get reimbursed for this type of expense, please provide:
- Documentation showing the person receiving service, date of service, type of service, provider name and the amount paid,
- Documentation showing the expense falls within the definition of basic preventive services as defined in Public Health Service Act Section 2713 (see list below), and
- Documentation showing the expense was not covered by a major medical health insurance or another health plan. This information is easiest to find on an Explanation of Benefits (EOB) you can get from your insurance carrier.
